What Is Spinal Cord Stimulation?

  • In simple terms: A small device implanted near the spine sends electrical pulses that reduce pain signals.

  • Scientifically: SCS involves percutaneous or paddle-lead implantation in the epidural space, connected to an implanted pulse generator (IPG). It modulates dorsal column activity, altering pain perception via gate control and central mechanisms.

Conditions Treated

  • Chronic pain syndromes

  • Post-surgical pain (failed back surgery syndrome)

  • Radiculopathy not relieved by surgery

  • Neuropathic limb pain

  • Complex regional pain syndrome (CRPS)

The SCS Process: Step by Step

1. Trial Phase

  • Leads temporarily placed in epidural space.

  • External stimulator used for several days.

  • If >50% pain relief achieved → proceed to permanent implant.

2. Permanent Implant

  • Pulse generator implanted under skin (abdomen or buttock).

  • Leads connected to generator; programming tailored to patient.

Benefits of SCS

  • Pain relief without further major surgery

  • Reduces reliance on medications

  • Reversible and adjustable therapy

  • Long-term relief for many chronic pain patients

Risks & Limitations

  • Lead migration or device malfunction (rare)

  • Infection or bleeding

  • Not effective for everyone (trial ensures selection)

Recovery Timeline

  • Trial: Outpatient, minimal recovery needed

  • Implant: 1–2 weeks for incision healing; full activity in 6 weeks

  • Long-term: Device programmed and adjusted for optimal results
